  • Complete and submit the online form
  • If applicable, complete payment online via Verisign's secure server. Please note, we accept Diner's Club, MasterCard, and Visa credit cards.
  • Once your transaction has been successfully completed, you will receive an email confirmation with a PDF version of your registration form.
  • We will then provide you with an application packet so you can move on to the next step in the process.

Complete my registration by mail
  • Choose to either:
    • Complete the online form, print it out and mail it to us with payment (if applicable)
    • Download, print and complete the PDF version of the form and mail it to us with payment (if applicable)
  • Once we receive your materials, we will send you an application packet so you can move on to the next step in the process.
