There is a fee of $16/person associated with this request - checks should be made payable to 'VT-Department of Motor Vehicles'

MEDICAL STATEMENT FORM
One for each member of the household - regardless of age

CIS Documents

NOTE: We cannot submit your request for an extension of your CIS approval until you have completed the Home Study update requirements!

Please keep in mind that these instructions are for CIS extensions only. If you are making any changes to your CIS status in terms of the number of children you are requesting, age range, special needs, change in country, change of primary residence, etc. you must contact your CIS Coordinator for further information. Additional CIS fees may be required for this type of request.

FINGERPRINTS: Fingerprints related to I-600A applications are valid for 15 months and cannot be renewed until the 90-day window prior to expiration. Lapses in fingerprint approval are okay unless travel is imminent or you are in need to extend your I-600A approval. CIS allows a one-time re-fingerprinting at no cost. Any re-fingerprinting requests thereafter will be subject to the $85/person Biometrics fee. You will need to select the second option on this request form, unless payment is required. For an extension of your fingerprints requiring a Biometrics fee, please contact the National Benefits Center directly or consult with your CIS Coordinator for how to do this.

I-600A EXTENSIONS WITHOUT A FEE: I-600A approvals are valid for 18 months and must be renewed within the 90-day window prior to expiration. CIS allows a one-time extension of an I-600A approval at no cost. An extension request thereafter will be subject to the $720 filing fee. Please be sure to consult with your CIS Coordinator if you are unsure as to whether or not a fee will be required.

I-600A EXTENSIONS WITH FEE REQUIRED: If you have already utilized your one-time no fee extension of your I-600A approval, an additional extension request will be subject to the $720 filing fee and $85/person Biometrics fee, and you will be required to complete a new I-600A Form. Please be sure to consult with your CIS Coordinator if you are unsure as to whether or not a fee will be required.

I-800A EXTENSIONS : I-800A approvals expire after 15 months from the time the first adult in the household is fingerprinted (the expiration date appears directly on the approval).
